Information for referrals

You are eligible for an osseointegration (click) prosthesis: if at least 2 of the following statements are affirmatively answered.

  1. prosthesis use: less than 50 hours per week
  2. maximum continuous walking distance with prosthesis: less than 2 kilometers
  3. release of prosthesis at least once a week during daily activities
  4. blisters, abrasions, pressure sores, inflammation due to the socket
  5. reduced seating comfort.
  6. excessive perspiration in the socket in the summer.

Contraindications

  1. smoking
  2. BMI >30
  3. severe diabetes mellitus (with multi-organ failure)
  4. severe chronic peripheral arterial disease in the legs

In the reference, state at least:

  1. patient data: name, date of birth, address, telephone number, general practitioner
  2. date of amputation, level and cause of amputation
  3. when available: radiographs, ct scans, vascular studies and daylight photos/videos of stump

Why a referral letter?

Health insurers have increasingly stringent requirements for reimbursement of specialist medical care. Article 14 of the Health Insurance Act stipulates that treatment can only take place if you have a referral from the GP, company doctor, medical specialist or other referrer accepted by your health insurer. If you do not have a valid referral, the health insurer will not reimburse the treatment. With this, the government wants to prevent people from using (more expensive) specialist medical care unnecessarily. Because of these regulations, the clinic will only schedule an appointment for you if you have a valid referral.

What conditions should the referral letter meet?

The referral letter should include at least the following information:

  • Patient data (name and address and date of birth);
  • Referrer details (Name, position and AGB code);
  • Description of the complaint for which you have been referred;
  • Date (Note: the date of the referral letter must be before or on the day of your first consultation otherwise the referral letter will not be valid).

Forgot your referral?

If you accidentally forgot to bring the referral letter, you can still have it faxed to you prior to the first consultation. If you do not have a valid referral letter, consultation with our specialists cannot take place. You will have to schedule a new appointment.

New referral?

Existing care needs

For follow-up appointments, you do not need a new referral each time. If the doctor indicates that you can contact him/her again if necessary, you will not need a new referral letter as long as the appointment:

  • is for the same complaints;
  • is made within 365 days of the last appointment. If the specialist indicates that you have been out of treatment for a complaint and a similar complaint returns, you should discuss with your GP again whether you can be referred.

New care needs

The specialist treating you cannot simply help you with another care need (new complaint). You will always need a new valid referral for this. It may not be clear whether you need to go to your GP again for a referral. In that case, you can always contact us. We are always willing to help you with this.

Second opinion?

If you are being treated by another specialist, but would like the opinion of one of AOFE Clinics’ specialists, your treating doctor or GP must give you a referral letter (note that this is not a referral letter for any treatment, only for a second opinion
