Information for referrers

You are eligible for an osseointegration (click) prosthesis: if at least 2 of the statements below have been answered in the affirmative.

  1. prosthesis use: less than 50 hours per week
  2. maximum continuous walking distance with prosthesis: less than 2 kilometers
  3. At least once a week release of prosthesis during daily activities
  4. blisters, abrasions, pressure sores, inflammation as a result of the socket
  5. reduced seating comfort.
  6. excessive perspiration in the tube in the summer.


  1. smoking
  2. BMI >30
  3. severe diabetes mellitus (with multi-organ failure)
  4. severe chronic peripheral vascular disease in the legs

In the reference at least state:

  1. patient details: name, date of birth, address, telephone number, GP
  2. date of amputation, level and cause of amputation
  3. when available: X-rays, CT scans, vascular examinations and daylight photos/videos of stump.

Why a referral letter?

Health insurers are setting increasingly strict requirements for the reimbursement of medical specialist care. Article 14 of the Health Insurance Act provides that treatment can only take place if you have a referral from a general practitioner, company doctor, medical specialist or other referrer accepted by your health insurer. If you do not have a valid referral, the health insurer will not reimburse the treatment. The government wants to prevent people from making unnecessary use of (more expensive) medical specialist care. Due to these regulations, the clinic will only schedule an appointment for you if you have a valid referral.

What conditions must the referral letter meet?

The referral letter must contain at least the following information:

  • Patient details (name and address details and date of birth);
  • Data of the referrer (Name, position and AGB code);
  • Description of the complaint for which you have been referred;
  • Dating (Note: the date of the referral letter must be before or on the day of your first consultation, otherwise the referral letter is not valid).

Forgot reference?

If you have accidentally forgotten to bring the referral letter, you can request it by fax prior to the first consultation. If you do not have a valid referral letter, the consultation with our specialists cannot take place. You will have to schedule a new appointment.

New reference?

Existing care demand

You do not always need a new referral for follow-up appointments. If the doctor indicates that you can contact us again if necessary, you do not need a new referral letter as long as the appointment:

  • is for the same complaints;
  • is made within 365 days of the last appointment. If the specialist indicates that you have been treated for a complaint and a similar complaint reappears, you should discuss again with your GP whether you can be referred.

New healthcare demand

The specialist treating you cannot simply help you with a different care question (new complaint). You always need a new valid referral for this. It may not be clear whether you need to see your GP again for a referral. In that case you can always contact us. We are always ready to help you with this.

Second opinion?

If you are being treated by another specialist, but would like the opinion of one of the specialists at AOFE clinics, your attending physician or general practitioner must provide you with a referral letter (please note, this is not a referral letter for any treatment, only for a second opinion).
